What to do if there's an accident, near miss or violent incident

Please also refer to the following guidance notes:

For accidents/near misses the Headteacher or nominated person should complete the online form within 24 hours of the incident.

Report a violent incident here

The reporting procedure applies to both council employees and non-employees (e.g. contractors, members of the public, pupils etc).

Once you've filled in the report form, a copy is automatically sent to the Headteacher, Health and Safety Unit and trade union (if RIDDOR and requested).

If the incident also needs to be reported to the Health and Safety Executive (HSE), as required under the Reporting of Injuries, Diseases and Dangerous Occurrences Regulations RIDDOR, please complete the relevant form on the HSE's site at http://www.hse.gov.uk/riddor/

When reporting under RIDDOR, please also provide the Health and Safety Unit with a copy of the report form and reference number.
